We have made requesting an appointment a simple process via our website. Please browse through our website and be sure to contact us if you have any questions or would like to schedule an appointment. You may also use our online appointment request form to schedule your appointment.
Insurance and Payment Options
For your convenience, we submit claims to your insurance company directly.
For Payment, we accept cash, debit and all credit cards. We also offer a flexible payment plans for large treatments. Please see our Financial Coordinator for details.
